Canvas Forms

Special Canvas requests such as creating non-academic courses, developing content, and requesting department course evaluations require manual administration.

All special requests must be made by completing one of the forms listed in this guide. Contact Dr. Ellen Bloomfield at ebloomfield@midway.edu if you have questions concerning this process.

What form do I need?

• UPDATE CANVAS COURSE CONTENT
  Use this form to update a Canvas course (Blueprint or other) with content from another course
• RE-OPEN COURSE FOR  STUDENT INCOMPLETE
  Complete this form if you have a student that needs extended access to a Canvas course to finish incomplete work.
• REQUEST CANVAS COURSE
  Complete this form to cross-list your courses or request a new canvas course shell
• REQUEST LTI TOOL
  Use this form to request an LTI Tool or textbook publisher materials.
• ROLES & PERMISSIONS
  Complete this form to request certain roles in Midway Canvas or specific courses
• REQUEST FOR TRAINING
  To request training, submit this form.
