forms iconSpecial Canvas requests such as creating non-academic courses, developing content, and requesting department course evaluations require manual administration.
All special requests must be made by completing one of the forms below. Contact Dr. Ellen Bloomfield at ebloomfield@midway.edu if you have questions concerning this process.

 


Course Requests

UPDATE CANVAS COURSE CONTENT

To update a Canvas course (Blueprint or other) with content from another course, please submit this form.  To ensure accuracy of update, please fill out all required information and use the naming conventions provided. 

 

RE-OPEN COURSE FOR  STUDENT INCOMPLETE

Complete this form if you have a student that needs extended access to a Canvas course to finish incomplete work.

 

REQUEST CANVAS COURSE

After you select Course Request type, you will be asked further questions on the form to fulfill the request.  Read each question carefully and provide the information needed in order to ensure accuracy of course creation and configuration.  The Center@Midway will create and configure the course as requested as soon as possible.  


Request LTI Tool

Canvas provides an option to include a variety of external tools to the application that instructors can then make available for their classes.  For your reference, these external tools are listed on the website EduAppCenter.com.  As an administrator of this site, The Center@Midway has created a process to determine which of these tools can be made available for use in Canvas courses.


Roles & Permissions

Canvas user role permissions allow users to access certain features and perform certain functions within a course site. A user's permissions within the course site are determined by the role the user is assigned.  All role requests must be approved by a Dean, Program Administrator, or Supervisor.

 
FERPA Training

FERPA training is required for roles that can view and edit student grade information. Any user who will hold one of these roles must complete the FERPA tutorial before being assigned to the role.  All Midway Faculty complete an annual Canvas training*. The FERPA training content is included in the Canvas training curriculum.   If you have questions about FERPA training completion, please contact Dr. Ellen Bloomfield (ebloomfield@midway.edu).
